Your purchase of the Foundational Assessment was successful.  

Before we can schedule your appointment, you need to complete the following steps:

1. Fill out the patient history and health questionnaire in Revelia
2. Log in to our Electronic Medical Record (Cerbo) for communication and to retrieve your lab requisition.
3. Go get your blood drawn at Quest, Labcorp or another lab location near you. 
4. After your bloodwork has been drawn, call us ASAP to schedule your Foundational Assessment.
