Your purchase of the Foundational Assessment was successful.  

Before we can schedule your appointment, you need to complete the following steps:

1. Fill out the patient history and health questionnaire in the Revelia software program.
2. Log into our electronic medical record Cerbo for all communication and to retrieve your lab requisition.
3. Go get your blood drawn at Quest, Labcorp or another lab location near you. 
4. When we receive your bloodwork results we will contact you promptly to schedule your Foundational Assessment appointment.
